forEVER SOPHIE - waiver
I acknowledge & agree to the following terms & conditions in relation to the permanent jewellery I am about to receive from forEVER SOPHIE.
I agree that SOPHIE has assessed the risks of this process & have put measures in place to mitigate. I agree to comply with the instructions of SOPHIE when participating in the process. However, in the unlikely event that I am injured, I understand this waiver prevents SOPHIE (including its employees and contractors) from any liability to the extent permissible by law.
The risks & measures taken are:
UV Light Damage from the welding machine- Provide appropriate PPE, & ensure it is always worn when the welding machine is being used. PPE is safety glasses.

Removal of Jewellery: I am aware that my forEVER piece can only be removed by cutting it off. SOPHIE can assist with this if necessary.
Warranty Period: I understand that SOPHIE offers a 2 month warranty but requires the broken chain to be returned for repair.
Durability & Care: While the jewellery is permanent, I acknowledge that it is delicate & must be treated with care. SOPHIE guarantees the weld, but not the chain or the jewellery itself against tarnishing, or other normal wear & tear.
Health & Safety: I confirm that I do not have a pacemaker or any other medical condition that could be negatively affected by the application of permanent jewellery. I understand the importance of this information for my safety & the safety of the technicians.
Allergy: I confirm that I do not have a sensitivity or allergy to any metals used in the jewellery, that is sterling silver, 9kt solid gold & 14kt gold fill with a brass base metal.
Liability: By signing this waiver, I release SOPHIE (including its employees and contractors) from any liability to the extent permissible by law for medical conditions or injuries that may arise as a result of receiving permanent jewellery or any experiences associated with it.
