forEVER SOPHIE - Waiver

I acknowledge & agree to the following terms in relation to the permanent jewellery service I am about to receive from forEVER SOPHIE.

Welding & Safety
I understand that permanent jewellery involves a welding process using heat & sparks. SOPHIE takes reasonable precautions to minimise risks, including the use of safety glasses & protective leather. I agree to follow all safety instructions & notify the technician if I experience discomfort or wish to stop.

Age & Children
Anyone under 18 must have a parent or legal guardian present & provide consent. Children must be supervised at all times & be able to remain still & cooperate during the fitting. Permanent jewellery is not recommended for babies or very young children, & jewellery contains small components that may present a choking or entanglement risk. We may decline or stop a service if we feel it cannot be carried out safely. The parent or legal guardian accepts responsibility for the child’s safety & jewellery.

Medical Conditions & Allergies
I confirm that I have disclosed any relevant medical conditions, implanted medical devices, known metal allergies or skin sensitivities. Jewellery materials may include sterling silver, 9kt solid gold & 14kt gold fill with a brass base metal.

Removal & Care
I understand that permanent jewellery can be removed by cutting the chain. I will not pull or tear it off. I acknowledge that the jewellery is delicate & must be treated with care.

Warranty
SOPHIE offers a 2-month warranty on the weld, subject to the broken chain being returned where requested. This does not cover loss, misuse, tarnishing or normal wear and tear.

Rewelds
If your permanent jewellery breaks or comes undone, please get in touch with us. We’ll assess whether the original chain can be rewelded. Rewelds may incur a fee, particularly if the chain or components need replacing. If the jewellery has been lost, a new chain will need to be purchased.

Acknowledgement
I confirm that I have had the opportunity to ask questions, understand the risks & aftercare requirements & voluntarily consent to the service. I agree to follow SOPHIE’s instructions & accept responsibility for risks arising from my failure to do so or from information I have failed to disclose.

Nothing in this waiver excludes or limits any rights I may have under applicable consumer law.
